World Championships Registration

Waiver and Privacy

Waiver & Agreement Form

In consideration of my acceptance as a member of the Age Group World Championship Team, and in order to compete at a World Championships, I hereby:

i. agree to wear the official Triathlon Canada team uniform unaltered, except as provided for by the Appendix B, section 7 of the ITU Competition rules, during the particular World Championships,

ii. agree to abide by the disciplinary and appeals procedures of Triathlon Canada,

iii. agree that as a member of the Junior or Age Group World Championship Team, I have the responsibility to and I shall clearly represent the interested parties (for example, sponsors), that my Age Group World Championship Team membership and World Championships accomplishments are achieved as an age-group and/or junior competitor,

iv. agree to conduct myself in a sportsmanlike manner at all times while attending and participating in the World Championships,

v. agree to attend all team meetings and check the official Team Canada Notice Board on a regular basis,

vi. agree to complete all medical documentation as required by the National Medical Director and advise such Director of any current medical conditions or treatments,

vii. agree to pay all required entry fees and uniform costs, where applicable, and settle all accommodation and other incidental costs when due,

viii. acknowledge that all fees are nonrefundable,

ix. acknowledge that I have sole responsibility for my personal possessions and athletic equipment at all times during this event,

x. acknowledge that as a member of the Age Group World Championship Team, I may be subject to unannounced  drug testing as provided for by Triathlon Canada’s agreement with the Canadian Centre for Ethics in Sport.

xi. consent to receive medical treatment which may be deemed advisable in the event of injury, accident or illness during, including immediately prior to and immediately following, an ITU Word Championship event,

xii. hereby attest that I am physically fit and have sufficiently trained for this competition, for my executors, administrators, heirs, next of kin, successors and assigns, waive and release any and all claims that I may have against Triathlon Canada, its officials, directors, members, volunteers and agents, Heartland Travel and their executors, administrators, heirs, and assigns, for all injuries or death suffered by me while in transit to/from or participating in, including immediately prior to and immediately following to the particular ITU World Championships.

And I understand that my failure to abide by the above conditions may result in disciplinary action by Triathlon Canada as provided for by the Disciplinary and Appeals Procedures.

Triathlon Canada Privacy Policy

Triathlon Canada is a registered non-profit Canadian Amateur Athletic Association that abides by Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA).  We require you, the athlete, to divulge certain personal information to Triathlon Canada in order for us to effectively administer your participation in national and international events.  However, we recognize that our members’ privacy is very important and we are dedicated to protecting your personal information at all times.


What Information is Collected?

Personal information is information that pertains to you specifically.  Once we receive your consent, we may gather personal information from you when you complete one of our application forms and send it to us by mail, fax or email.  The type of information we may collect from you includes:

  • Name
  • Mailing address
  • Telephone number
  • Email address
  • Date of birth
  • Gender
  • Provincial Triathlon Federation membership number & Expiry date

The choice to provide Triathlon Canada with the aforementioned personal information is always yours.  However, your decision to withhold some of these details may limit our ability to register you for certain races or events.  If we are unable to accommodate your request based on the information that has been provided, we may ask for additional details in order to identify other ways to be of assistance.

How is Your Information Used?

When may information be disclosed to outside parties and to whom may it be disclosed?

Triathlon Canada is obligated to keep your personal information confidential except under the following circumstances:

  • When required by law
  • When permitted by law
  • When authorized by you

When Required by Law
In some cases we may be required to disclose certain information to authorities.  This requirement may pertain to a legal proceeding, a court order or to tax reporting requirements.  We will disclose only the information specifically requested and we will take precautions to be certain that the authorities who are requesting this information have legitimate grounds to do so.

When Permitted by Law
There are instances where Triathlon Canada is legally permitted to disclose personal information such as collecting a delinquent account, a medical emergency or suspicion of illegal activities.

When Authorized by You
Your signature on this form authorizes Triathlon Canada to disclose the aforementioned personal information to the International Triathlon Union and World Championship race organizers.  You are also acknowledging that Triathlon Canada employees and volunteers will have access to this information in the normal course of daily National Office operations and Race Day operations.  Triathlon Canada will not disclose your personal information to any other third parties beyond those described above.
